If your cell phone rings during class, you must write a formal apology directed to the class.

In the space provided below write an apology to the entire class with at least 150 words. You are responsible for making enough copies for everyone in the class including one for my records. If this occurs more than three times during the course, you will receive a 5% reduction in your overall grade in the course. Failure to properly fill out the form will also result in a 5% reduction in your overall grade in the course. If your phone rings during the final exam, you will lose 10points on the exam. Exceptions may apply if the student has prior approval because of a medical/family emergency.
